Please complete this form if your child's jersey is damaged due to a chipped, cracked, or pealing uniform number (located on the back of the jersey) that is not the cause of normal wear or wash.  Complete one form for each of your children (if needed). 

The form must be completed by November 6, 2017. 

Replacement jerseys will be delivered at the start of Spring season in March 2018.

Please note, only the jerseys will be replaced (not the shorts or socks).
